Patient Forms

Once you have been registered and scheduled with a clinician you can either choose to complete these forms on the patient portal or download these forms to complete them.

Helpful Information

Patients (or parents/guardians of minor children) are responsible for all services provided. We will submit insurance claims on your behalf unless you request otherwise. Copayments and deductibles are due at the time of service. Billing a non-custodial parent or third party requires written authorization.

Appointments canceled with less than 24 business hours’ notice, or missed appointments, may be charged the full session fee and cannot be billed to insurance.

Most providers offer evening, weekend, or early morning appointments. Administrative hours are Monday through Friday, 8:30 a.m. to 5:30 p.m.

In the event of an emergency at any time, please call 911 or go to your nearest emergency room.

For after-hours needs:

  • If your concern cannot wait until the next business day, please call our after-hours service at 440.473.9525.
  • If your symptoms have been ongoing or you need a medication refill, please wait until the next business day to contact the office.
  • If leaving a message, remain available by phone so we can reach you.
  • Prepare your questions in advance and have your pharmacy’s name, phone number, and address available.
  • Keep paper and pen nearby to note your clinician’s instructions.
  • Whenever possible, patients should call on their own behalf. Privacy laws require a Release of Information for another adult to call for patients 18 and older.

Please note that consultation calls after hours may incur a charge.

Medication is prescribed only after a thorough evaluation. Following an initial prescription, regular follow-up appointments with your psychiatrist or nurse practitioner are essential to ensure appropriate dosing and monitor side effects.

Prescriptions are provided only in coordination with scheduled office visits. Refills are issued during follow-up appointments. Urgent refill requests without a visit may incur a $25 administrative fee (not reimbursable by insurance). Please allow 48 hours for processing and contact your pharmacy directly for prescription status updates. Refill requests initiated by pharmacies will not be honored.

Psychotherapy is a collaborative relationship between the patient and a skilled practitioner focused on meaningful change in thoughts, feelings, and behaviors. Both therapist and patient share responsibility for establishing goals and making therapy effective. Patients are encouraged to be open and honest, attend scheduled appointments, and follow through with agreed-upon work between sessions.

Psychological testing uses specialized tools to assess areas such as intelligence, academic achievement, personality, and perceptual or motor functioning. Testing is typically conducted one-on-one with a psychologist, though some measures may be self-administered. Results are individually analyzed and reviewed with you. Honest and thoughtful participation is essential for accurate results. Please note that insurance coverage for testing and testing materials is often limited.

Insurance FAQs for New Patients

We participate in most insurance plans. Patients are responsible for contacting their insurance provider to obtain authorization, confirm clinician participation, determine copayments, and understand any session limits. Insurance reimbursement requires consent to release information related to your care.

Insurance benefits can change at the beginning of a new year or when your plan renews. You may have a new insurance provider or plan, or your deductible or out-of-pocket costs may have changed. Because your financial responsibility is determined by your agreement with your insurance company, we strongly encourage you to contact your insurer directly to fully understand your coverage and potential costs.

Coverage for virtual visits varies by insurance plan. To understand your benefits and any out-of-pocket responsibility, please contact your insurance company and ask whether virtual visits are included in your plan and what costs may apply.

A deductible is the amount you must pay out of pocket before your insurance begins to cover services. Deductibles typically reset at the start of each calendar year. If your plan includes a deductible, you may be responsible for the full cost of sessions until it is met. Many patients can expect to pay approximately $65–$100 per session until their deductible is satisfied, depending on their plan.

A co-payment is a fixed dollar amount required by your insurance company for each visit. Some plans use the terms co-pay and co-insurance interchangeably, though they are different (see below).

Co-insurance is the percentage of costs shared between the patient and the insurance company after the deductible has been met. For example, your plan may cover 80% of the cost, while you are responsible for the remaining 20%.

The out-of-pocket maximum is the total amount you are required to pay toward healthcare costs during a calendar year, excluding monthly premiums. This may include deductibles, co-payments, and co-insurance.

Once you reach your out-of-pocket maximum, most insurance plans will cover 100% of medically necessary services for the remainder of the year. Please note that some plans do not count all expenses toward this maximum, so it is important to confirm details directly with your insurance provider.

A Coordination of Benefits (COB) denial occurs when your insurance company needs to confirm whether you have additional insurance coverage and which plan is primary. Claims cannot be processed until this information is verified by the policyholder.

If you receive a denied Explanation of Benefits (EOB) or a bill for the full amount of a visit, please contact your insurance company as soon as possible to resolve the issue. Until COB is completed and claims are processed, balances may remain the patient’s responsibility.
